As both an Individual Medical Evaluator and a Qualified Medical Evaluator, our doctors are often called on to work with Worker’s Compensation cases.

If you are a Worker’s Compensation patient and dental treatment is part of your settlement, talk with your attorney about more information regarding our services.

Before you come and visit us, we ask that you fill out some paperwork in order to best treat your needs. The packet will be sent out to each patient.

Please fill out as much as you can to the best of your ability. We understand that in some cases, the paperwork may get lost. Just let us know, we have more here at the office!
